The VAC-WIL Student Evaluation Form is designed to be used by both students and employers to evaluate performance and work done during the placement.
Student Evaluation Form
This document can be used by completing these three easy steps.
Step 1
The student completes a self-evaluation by filling out the evaluation form. The areas designated for the supervisor to complete will be left blank.
Step 2
The student sends the evaluation form to their supervisor to complete.
Step 3
Once the supervisor has completed the evaluation, they set a time with the student to review the evaluation together.
